Wrongly-rendered PDF

Hi,
I need to generate every month on a french administration website PDF files to give to people I employ.
The generated PDF files do not render completely properly in Okular, which uses poppler as rendering lib, while they do in xpdf and acrobat reader : some black marks, indicating the choice among options, are not rendered.
I'm going to attach a file for you to check by yourself.

When rendering the pdf with poppler 0.12 i don't see anything evidently wrong in the pdf, can you either:
 a) post a image saying what is wrongly rendered
 b) upgrade to poppler 0.12 to see if the problem goes away

With latest poppler 0.48.0 and okular 0.26.70, it seems the rendering is fine, but none of the fields can be changed.

The forms are not editable either in Acrobat Reader, so i guess that's fine, closing this as fixed unless we get a pointer to what is wrong with it.
