In the Spelling and Grammar dialog box, if a certain misspelled word appears more than once, even if you add the first instance to say standard.dic, it will still prompt you for each additional instance of the misspelling, for a given section of text that is presented in the dialog.

But on subsequent sections of text shown, it will behave as expected, that the word has been added to standard.dic, so is not prompted as misspelled.
