Reproducing steps:
First Xorg start is ok, but restart fails with GPU hung with very high probability

You have very old user space and a very recent kernel.  Is this a recent regression?  If it is, are you able to bisect the kernel to figure out when then failures began?

I started from 2.6.32 and found the same situation here, After restart screen becomes dark, there is only mouse cursor at the center (movable). But syslog look differ, there is no GPU hung. I attach it.
