Confirmed under Windows XP SP3 with LO 4.0.0.O.beta2. Set to NEW according to comment #1. In my tests, this problem results broken conditional content: hiding sections doesn't work, but this function is critical for template management of public administrations (like Munich) and other organizations. OK gets called twice, my mistake from non-restarting-dialog-apply fix attempt I bet Caolan McNamara committed a patch related to this issue.
