DVI monitor is connected to HDMI port via HDMI->DVI adapter, if it matters.

Does it display correctly with

Section "Device"
        Identifier "default"
        Option "Accel" "off"
EndSection

in /etc/X11/xorg.conf? Please attach the corresponding Xorg log file.

…and I've changes pci-cards inserted into MB and the issue is no longer reproducible.
Unfortunately I don't remember what and where exactly was inserted and during quick plug/unplug session I wasn't able to reproduce it again.

Closing for now.
