Trying to complete a state tax form that was created or saved (I don't know which) on a Mac using Mac Preview.  However, none of the input fields are visible nor can they be selected (it's like they aren't there).  The maintainers of the form have used Windows and Adobe Reader to load and re-save it, so I can't provide a link to the failed form.  However, I have attached my copy of the form for your use.
Linux Mint 17.3 and Document Viewer 3.10.3

FWIW, it does not seem to be a regular form (AcroForm or XFA), it is implemented with annotations (widget).

It cannot be filled with Evince 3.28 nor Okular 1.3.3.
