Stream_Sending_Change_Reason should have a "ChangeSucessful" reason or something, it's confusing to call it with UserRequested from tp-fs to for pending -> not-pending transitions

Also, in the description of LocalSendingState in Call.Stream this line should be removed: "When a call is accepted, all initial contents with streams that have a local sending state of Sending_State_Pending_Send are automatically set to sending."

The reasoning is that only tp-fs should change from Pending_Send to Sending when it has managed to open the device.
