patch to add a -singlefile option to pdftoppm

This patch adds a -singlefile option to pdftoppm to write only the first page and not to append digits to the file name.
This is useful when generating bitmap previews of PDF files with a script when you want the preview to have the same name as the PDF (except for the extension).
When used with -f #, this option will process the specified page.
