The pdf is created using GNU Lilypond 2.4.5, a music typesetting program that uses Ghostscript for pdf creation. Xpdf and other pdf viewers render it correctly. I am pretty sure this is a freetype bug. I expect you have freetype 2.1.7 which is broken. It should be fixed with the newer versions of freetype. If you can reproduce with a newer version of freetype reopen it.
