isn't shown correctly in Evince: it pretends to have done rendering pages, but they are all blank. Okular has no problems with this file, so I suspect poppler-cairo.

I'm using:
poppler 0.14.3-0ubuntu1.2
evince 2.32.0-0ubuntu1.1

Confirmed with poppler-cairo 0.16.7 and Evince 3.0 (Fedora 15).

Reproduced with 0.16.7 but works fine with master. Probably one of the many bugs in the cairo backend that have been fixed since 0.16.7.
