The DVI output on most Radeon 9200/9250SE based cards simply doesn't work using the radeon driver. This problem is there for more than a year now and has never been fixed. VGA output works fine though. I verified that this is not an issue of a wrong configuration file, as the same configuration works for one 9200 card but not for another one. This must be due to a different layout or chip variant. Lowering the resolution, reducing hsync, vsync, nothing helps to circumvent this problem.
There is a good chance that this is simply a hardware issue. Some of the R200-class video cards were manufactured with reportedly bad DVI components; I would suggest lodging a complaint with ATI.
