i965: Use state streaming on programs, and state base address on gen5+.

causes gpu hang.

try to run any game like http://maniadrive.raydium.org/ or GPU caps viewer with WINE except glxgears.

anyway, off topic - why Linux Intel driver is so slow :(

Hmm, if that commit is truly to blame (which at first glance it should be because gen4 is excluded from the changes), then it is likely to have left a tell-tale in /sys/kernel/debug/dri/0/i915_error_state.
