Integrated database.
Integrated HSQL
The wizard works for creating forms on tables, but after trying to save the form, it doesn't appear to be anywhere.
Opening the same database with LO 3.3 on anther PC : no forms present.  3.3 creates then OK.
When reopening the database with 3.4.1 on the mac, still no FORMS.
The tables are OK, no loss of data.

Have you reproduced the bug with 3.4.4 (current version) ?

As for me, I haven't reproduced this with 3.4.4 but I'm on Debian x86-32 testing.

started right out as NEW without ever being explicitly confirmed. The bug is

are you using 3.5? The last stable version is 3.5.5. Is bug still in 3.5.5?

please feel free to reopen this bug if you find out that the problem still exists with the current stable LibreOffice version and if you can contribute requested additional information due to (especially BugReport Details)! 

Set status to "RESOLVED -> WORKSFORME"
