When going into Tools --> Options --> Appearance --> Application Background and changing the colour to something other than the default in Libreoffice Draw, nothing happens.

When doing the same thing in Writer or Impress, the application background changes colour (the desired behaviour). 

I also have noticed this problem in the Libreoffice 3.4 series.

reproduced in 3.6.0 master
But my be exist some reason for this. In Draw background always is white. IMHO it is more handy to draw or paint on white.

Confirmed with:
LO 3.5.5.3 
Build ID: own W7 debug build
Windows 7 Professional SP1 64 bit

Application background unchanged. Seems it is linked with document background option.

When I change in Format->Page, tab "Background", nothing changes outside of page

It works for me:
Version 3.6.0.4 (Build ID: 932b512) Slackware Linux 13.37

Closing as this is an old bug and I suppose this is fixed in newer version. If you think otherwise please re-open it.
