sample PDF form

I have tried editing the attached PDF in both evince and okular.

I can change the name of the company in section 5 (Consignee name and complete address), but this information is not successfully saved when I save the PDF.

In okular, the new data is lost, and the old data is shown.

In evince, the new data appear if you select the field, but the old data is shown if you do not.

Works fine in latest okular+poppler from git
