module-bluetooth-device doesn't monitor the stream fd for hangup or errors when using the SCO over PCM mode. The stream may get closed by bluetoothd, for example, when the user requests the audio to be moved away from the bluetooth device (a feature of the HFP profile). When not using the SCO over PCM mode, the current behavior is to set the bluetooth card profile to "off". That should be done also with the SCO over PCM mode.
