Extension : PDF import 1.0.5 in LibO 3.5.1.2/Win XP SP3/Java 1.7.3. Same problem with LibO 3.4

Problem description: 
The extension "PDF import" displays two cheboxes instead of one in a PDF document when opened.

Steps to reproduce:
1. Download cerfa_13404.pdf (see attached file)
2. Open it with LibO
3. All checkboxes in the document are displayed twice (p.2 for exemple)

Current behavior: two checkboxes instead of one

Expected behavior: checkboxes displayed only once

PDF from link is corrupted. Please, attach correct version of this file. 
Attaching of files bigger than 3 Mb is impossible. Tell if this file is bigger than 3 Mb.

Bug seems to be solved in LibO 3.5.4.2/PDF import 1.0.5

Thanks for additional testing and for attachment
on 3.3.4, 3.5.4, 3.6.b1 check-boxes also looks correct for me 

Due to last comment, changing status to WorksForMe

If problem will appear again, please, change status to Reopened
