first page printed as pdf in evince

After applying fixes from latest git I still have a problematic document which is missing characters when printed as pdf or on paper using evince.

First page printed with evince as PDF is attached and misses "ff" in the first heading.

I currently use:
cairo latest git 5dd0f2eb9   (1.12.2 + 127 commits)
pixman latest git g1dcca0f  (0.27.x)
poppler 0.20.2
evince 3.5.4

I don't know if this is still cairo related or if I just miss something else. All fonts are embedded. So please try to reproduce and let me know what else I can provide. 

Downgrading to cairo 1.10 did not seem to fix the issue this time.
