I can reproduce this problem (with the attached files) in Version 4.0.0.0.beta2 (Build ID: 4104d660979c57e1160b5135634f732918460a0) on Ubuntu 12.04 (x86)
for me not reproducible with LO 4.3.2.2 (Win 8.1) Does this issue still persist for you or anybody else in Linux with the latest release of LO?

Since no reply to Comment 3 and I can't reproduce in Ubuntu, I change to Needinfo. What exactly does it mean "all pictures are in the wrong place"? Screenshot looks fine, like a document.
im using 5.1.2.2 now and i dont have this problem ennymore. also no problem in the last few versions.
Then it's WFM, since no fix commit is known.
