It looks like the EZH isn’t treated as a majuscule by the Single Substitution - Case Form. Which I think means it needs to be added to Class 1047. I don’t see how to do that via the GUI, but it is a simple text edit to the .sfd files, if my diagnosis is accurate. If so, there may be other majuscules missing from that table.
