Although Evince shows the file attachments in some PDF's, it does not in this
particular PDF (which I created).  The attachment shows up just fine in Acroread.

Sorry.  I thought putting the URL for the "non-working" PDF in the URL box would
provide it.  It doesn't seem to though.  Therefore, here's the example PDF.

The problem is that the attachments aren't at the top level - they are on pages 2 and 3.
