Hrm, I guess the 0x3d4 vga writes aren't such a big hit on this card since it doesn't have a PDISP at all either. Would it be possible for you to bisect nouveau to see what change broke nouveau for you? git bisect start v3.16 v3.13 -- drivers/gpu/drm/nouveau should only bisect the nouveau commits and not the whole kernel.
I am not familiar with git so I started to test official kernel sources from kernel.org. 3.14-rc1 have broken nouveau driver for me. Unfortunatel 3.13.0 also contains bug with filesystem or HDD or something similar. I think he have the same bug as me: https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1380881 So I can not test a lot of 3.13* kernels :/ As far as I remember Ubuntu with 3.13 *something worked for me (both filesystem and nouveau).

Can you include your vbios (/sys/kernel/debug/dri/0/vbios.rom)? I suspect that something in the vbios is telling nouveau to write to the VGA registers, which aren't there.
