Hardware: Macintosh Powerbook G4, Mobility Radeon 9600 M10

I can only use the second head on this Mac if the monitor is plugged in at boot
time. If I plug the monitor in subsequently, it is not detected at X startup. If
the Mac is put to sleep, then the output to that monitor is not initialised on
resume, even if it was working beforehand (eg, with a working Xinerama setup,
the laptop display will resume & X itself will resume as if all is well, but no
output will appear on the 2nd head).

External TMDS chips needs proper init.
