Certain boxes that are fillable in acrobat are not fillable in evince(or okular, or atril):

Boxes 1, 2 and 8 on part 5 are not fillable.

It happens with another form:

http://www.uscis.gov/sites/default/files/files/form/g-28.pdf

I think this is related to boxes becoming editable or populated depending on another box changing.

For the G-28, you must make a selection in one of the 3.b boxes for part 1 before the text box becomes editable.

This works with acroread, but not with poppler programs.
